Providing customized care for the individual.
Home visits constitute the core of services offered at the UAY Teen Parent Program, allowing services to
be "customized" according to participant needs. Annually a Pediatric Nurse Practitioner and Family Support Workers provide
approximately 1,000 home visits promoting child development, health, and parent skill enhancement.

Educational Home Visits: Parent skill building is offered on home visits, in classes,
and in support groups. Special emphasis is placed on the parent's role on healthy brain growth and development during
the early years of life, creating nurturing home environments, child development, positive discipline, role modeling and age
appropriate expectations.

Health Home Visits: Recognizing that children of teen parents are at greater risk
for being born low birth weight, having more health problems and lagging behind developmentally, a Pediatric Nurse Practitioner
provides pre- and post-natal home visits to teen parents and their children. Services provided include: nutrition, healthy
pregnancy and child birth education, well-baby checks, developmental screenings, and anticipatory guidance.
